Family tree software records names, dates, and lineage. A genogram maker records those things too, but that is roughly where the overlap ends. The notation system developed by Monica McGoldrick and Randy Gerson in 1985 added a layer of clinical meaning to the diagram: not just who is related to whom, but the quality of those relationships. A double line between two people signals an overly close bond. Slashes across the horizontal marriage line indicate separation or divorce. Three parallel slashes signal a cut-off. None of those distinctions exist in genealogy software because genealogy software was never designed for a therapist trying to understand why a client defaults to the same conflict pattern in every relationship they form.

Medical history tracking is the second major divergence. A genogram maker built for clinical use divides each person symbol into quadrants, each mapped to a category of condition: cardiovascular, psychiatric, oncological, substance-related. When a supervisor looks at a completed genogram and sees the same quadrant shaded in three consecutive generations on the maternal side, that is clinical information. A family tree shows the same three people with the same dates; it cannot show the pattern.

Relationship type coverage is the third difference. The McGoldrick-Gerson-Petry 4th edition (2020) defines 22 structural relationship types and 38 distinct emotional overlays. Tools built for general diagramming or genealogy typically support 10 to 15 relationship types at most, which is enough to draw a family but not enough to document the fused, hostile-dependent, or cutoff dynamics that make a genogram clinically useful. The notation standard exists precisely because shared symbols allow one clinician to hand a completed genogram to another and have the second clinician read it without explanation.

For students in LMFT, MSW, nursing, or clinical psychology programs, learning to use a proper genogram maker matters beyond completing assignments. Supervisors and licensing boards expect practitioners to produce diagrams that conform to the McGoldrick standard. A genogram drawn in a generic charting tool or a slide deck application may look plausible to a layperson but will lack the symbol vocabulary a clinical reader expects.
